Release & Waiver of Liability

In consideration of being permitted to participate in movement, wellness, and mindfulness
classes, including yoga and Pilates, as well as workshops, events, sound baths, and related
activities (collectively, the “Programs”) offered by Sanctuary Collective LLC, I acknowledge and
agree to the following:

1. Assumption of Risk
I understand that participation in the Programs involves physical movement, breathwork,
and other activities that carry inherent risks, including but not limited to physical injury,
illness, aggravation of pre-existing conditions, slips or falls, equipment failure, and in rare
cases, serious injury or death. I knowingly and voluntarily assume all such risks, whether
known or unknown.

2. Health Responsibility
I affirm that I am physically able to participate in the Programs and have consulted, or
will consult, a physician as needed regarding my participation. I understand that it is my
responsibility to listen to my body and to modify or stop participation at any time.

3. Medical Disclosure
I agree to inform Sanctuary Collective LLC of any medical conditions, injuries, surgeries,
or pregnancy that may affect my ability to participate safely.

4. Release of Liability
On behalf of myself and my heirs, assigns, and legal representatives, I release and
waive any and all claims against Sanctuary Collective LLC, its owners, instructors,
employees, contractors, affiliates, and landlords for injury, illness, loss, or damages
arising from my participation in the Programs, including claims based on negligence.

5. Indemnification
I agree to indemnify and hold harmless Sanctuary Collective LLC and the parties listed
above from any claims or demands arising out of my participation in or presence at the
studio.

6. Right to Refuse Participation
I understand that Sanctuary Collective LLC reserves the right to refuse participation at
its discretion and that my sole remedy may be a refund.

7. Cancellation & No-Show Policy
I acknowledge and agree to Sanctuary Collective LLC’s cancellation, late-cancel, and
no-show policies, including applicable fees. Late cancellations and no-shows may result
in loss of class credit or charges to the card on file.

8. Age Requirement
I confirm that I am at least 18 years of age, or that a parent or legal guardian has
provided consent where required.

9. Personal Property
I understand that Sanctuary Collective LLC is not responsible for lost, stolen, or
damaged personal belongings.

10. Media Notice & Consent
I understand that Sanctuary Collective LLC may occasionally take photographs, video,
or audio recordings during classes or events for promotional, educational, or marketing
purposes. I acknowledge that advance notice will be provided when such recording is
planned, and I will have the opportunity to opt out if I do not wish to be included.
By participating after notice is given, I grant permission for my likeness or voice to be
used by Sanctuary Collective LLC without compensation.

11. Communicable Disease Acknowledgement
I acknowledge the risk of exposure to communicable illnesses, including COVID-19, and
voluntarily assume all such risks. I release Sanctuary Collective LLC from any claims
related to illness or exposure arising from my participation.
